Empowering Disabled Consumers Through Personal Health Records Ron Manderscheid, Ph.D. Constella Group, LLC & Johns Hopkins University

Three Major Points SCIENCE: Supports empowerment and the application of technology to PHRs for disabled consumers. CASE STUDY: Supports empowerment of disabled mental health consumers and PHRs for their use. BUSINESS CASE: Supports a specific focus on PHRs for persons with disabilities.

The Science

Institute of Medicine Framework “Crossing the Quality Chasm”

Institute of Medicine Framework “We have a large and growing chasm between actual healthcare delivery and the quality of the healthcare we need.”

Institute of Medicine Framework Key principle: “The patient is the ‘true north’ of the health care system.

Institute of Medicine Framework Key service goals: SAFE PERSON-CENTERED EFFECTIVE EFFICIENT EQUITABLE TIMELY

Institute of Medicine Framework Necessary Strategies: Strategy 4. Using Information Technology

The Case Study

Mental Illness Population Mental and Substance Use Conditions: Any Disabling Adults 25% 6% Children and Adolescents 20% 9-13%

Mental Illness Population Those with disabling conditions use approximately 80% of all financial resources expended for mental health care.

Mental Illness Population Specialty Care: $120 Billion/Year ($400 per American) State Care (All Departments): $100 Billion/Year SSI/SSDI: $19+ Billion/year Medicare: $41 Billion/Year (Exclusive of Pharmacy)

President’ New Freedom Commission on Mental Health Key principles: Foster consumer and family direction of care Foster recovery-oriented care

President’ New Freedom Commission on Mental Health Key goals: Goal 6. Technology is used to access mental health care and information.

The Business Case

Personal Health Records Persons with disabling conditions need personal health records to: Monitor health status Monitor health care use and quality Learn better approaches to self-care Monitor the expenditure of health care resources Maintain virtual contact with other consumers

Personal Health Records Persons with disabling conditions are highly motivated to: Monitor their personal health status Engage in effective self-help Achieve the best results from health care

Personal Health Records This attention to self help and effective care can pay financial dividends: Effective self-help reduces health care costs. High quality, effective care reduces the need for costly additional visits. Self-coordination of care reduces costly fragmentation.

A Hypothetical Care for chronic illnesses now costs in excess of $1 trillion per year. A savings of only 5% percent through PHRs for persons with disabilities would pay a dividend of $50 billion each year.

Bottom Line Empowering persons with disabilities through PHRs will lead to improved quality of care and reduced costs.
