Assessment of Community Newaygo County 2009-2014 More sedentary lifestyle Increased rate of obesity Increased death rates Increased teen pregnancy Increased smoking & drinking Lack of routine Dr. visits Lack of leisure time activities

Analysis of Assessment Findings Social Detriments Of Health (2014) • Jobless rate of 6.8% reported • 17.9% persons below poverty • 58.4% of children must have free or reduced lunch • 57.7% Medicaid paid births • 2545 people per PCP (double the average in MI) • 2180 people per dentist (higher than MI average) • 1547 people per mental health provider (almost three times the MI average) • 22.8% of residents had no access to healthcare in the past year (r/t cost) • 26.4% smokers, 41.8% obese, 53.3 per 1000 teen pregnancy, and 10.6% diabetes. (DHD #10, 2014)

Problem Statements Identified! Increased risk/rate of diabetes within Newaygo county adults. Increased rate of binge drinking incidence among adults Increased sedentary lifestyle and obesity among adults and children

Community-Focused Nursing Diagnosis Increased risk of death related to lack of knowledge of diabetes as evidence by community obesity and increased death rates.

Goals and SMART Objectives Decrease rate of death r/t diabetes Decrease new onset of diabetes Decrease glucose level for already diagnosed individuals Increase physical activity community wide Increase healthy eating options Increase healthcare seeking behaviors/ routine PCP visits Increase education on diabetes awareness SMART Objectives (Specific, Measurable, Achievable, Relevant, Time oriented) Community will show interest in increasing physical activity by attending workout sessions within the first month of launching. Community will ask questions about diabetes education and show interest in lowing glucose level within the first month of awareness activities. Community will be receptive of healthy eating options offered at restaurant locations and show interest by ordering diabetes friendly foods within three weeks of initiation. Community members will schedule/attend routine PCP visits within the first year and follow guidelines for adequate healthcare.

Policy Implications If this initiative is carried out effectively, it stands to save Newaygo County some money. If obesity rates do not increase, our country “will save $550 billion dollars by 2030” (“About the United States Healthful…”, ushfc.org). Newaygo County could see relative savings. Newaygo residents will be healthier and happier. Newaygo County will become a “destination” for young professionals and retirees. Money that would have be spent on disease maintenance will instead be directed towards disease prevention research and education.
