Paperwork: Every student who goes abroad on a semester exchange, affiliated, or non-affiliated program must complete paperwork for IC and upload to online OIP application file. Release Form Consortium Agreement Form (SFS use only; do not need to upload to your file) Petitions for Assignment/Transfer of Credit Study Abroad Policies & Procedures Form Plus completion of various electronic forms online Students going on non-affiliated programs must also take a leave of absence. All forms (paper/electronic) must be uploaded or completed no later than 5/5/17
