There is need to study the variation in the quality of post graduate training in the tertiary care hospitals of Karachi. Objective: To assess the quality of postgraduate fellowship training in four leading teaching hospitals of Karachi, Pakistan. Study design: Cross sectional survey. Place and Duration of study: The study was conducted in four leading public and private sector hospitals of Karachi. These hospitals included Aga Khan University Hospital (AKUH), Ziauddin Medical University Hospital (ZMUH), Civil Hospital Karachi (CHK) and Jinnah Postgraduate Medical Centre (JPMC) from January to March 2012. Methodology: A total of 246 postgraduate fellowship trainees were interviewed. A questionnaire adopted from CPSP guidelines on postgraduate training standards was developed in which postgraduate residents were asked to rate educational training and supervision on 5 point agreement likert scale. It includes domains of educational practices, balancing service, training and life, resources for clinical practice and medical education and supervision, assessment and feedback mechanism. Postgraduate fellowship trainees (PG’s) were selected on the basis of convenient non- probability sampling methodology. Fisher's exact test and chi-square test was used to find differences in quality of postgraduate training among four teaching hospitals of Karachi. Percentage of agreement and comparative comparison among four teaching hospitals of Karachi Results: The mean duty hours per week was highest in private sector hospital AKUH i.e., 86.70 as compare to mean duty hours of government sector hospital CHK which was lowest i.e., 52.71 and their difference among all four hospitals was found to be significant (P-value <0.001). Around 85.2% of the PG’s in AKUH agreed that they have a structured study program as compare to 32.8% of PG’s in CHK which was lowest and their difference among four hospitals was found to be significant (P-value <0.001). Around 51.2% of the PG’s in JPMC agreed that they spent appropriate amount of time with their family and friends as compare to 0% of PG’s in AKUH which was lowest and their difference among all four hospitals was found to be significant (P-value <0.001). Around 74.1% of the PG’s in AKUH agreed that the hospital provides them adequate and appropriate clinical supervision as compare to 36% of PG’s in ZMUH which was lowest and their difference among four hospitals was also found to be significant (P-value <0.001). Conclusion: There is need to standardize the postgraduate fellowship training in Pakistan.
