An Interconnected Systems Framework A Structure and process for education and mental health systems to interact in most effective and efficient way. guided by key stakeholders in education and mental health/community systems who have the authority to reallocate resources, change role and function of staff, and change policy.

Core Features Aligned Effective teams that include community mental health providers, family/youth Data based decision making Formal processes for the selection and implementation of evidence based practices (EBP) Early access through use of comprehensive screening Rigorous progress-monitoring for both fidelity and effectiveness Ongoing coaching at both the systems and practices level.

Weaknesses based on Survey... 1.2 Leadership from Community Mental Health or Child/Youth Service Organization participates on the District and Community Leadership Team. These individuals have the authority to make key decisions regarding policy, funding, leveraging resources, and providing support to the ISF implementation Action Plan... ?

Weaknesses based on Survey... 1.5 Full school staff and faculty are trained on ISF. Action Plan... Provide training opportunities for the remainder of the staff.

Weaknesses based on Survey... 1.4 Based on results from resource mapping and screening data, schools implement a school-wide mental and behavioral health intervention at the Tier 1 level. Action Plan... Attending the Hershey forum “If you screen, you must intervene.”

Weaknesses based on Survey... 1.9 Families’ perspectives of ISF implementation are solicited regularly (annually) through a variety of methods (surveys, forums). Action Plan... Develop survey to send to families Send newsletters to inform families of SWPB happenings

Weaknesses based on Survey... 1.13 Universal mental health or social-emotional screening is conducted in the areas of both internalizing and externalizing concerns. Action Plan... Recently, select staff attended Mental Health and First Aid Training Continue to train remaining staff on Mental Health and First Aid

Weaknesses based on Survey... 2.1 Tier 2 is in place as measured by scores on the BAT or Tier 2 TFI. Action Plan… Review data from ISF survey and implement action plans/strategies to address areas for growth.

Weaknesses based on Survey... 2.5 Families whose students are receiving Tier 2 Interventions participate in the intervention. This may include (1) reinforcing progress at home, (2) learning key intervention components, (3) implementing key intervention components at home, and (4) regularly communicating with the school and/or community employed staff about progress at home.

Action Plan… Develop incentives or motivational plans to engage reluctant family participation in Tier 2 interventions. Positive interaction with families/students. Encouragement, guidance, and genuine care for families and students.

Weaknesses based on Survey... 2.14 Multiple sources of school and community data are used to determine need and fit when the Tier 2 building system conversation/team is selecting mental and behavioral health interventions. Action Plan… Collaborate with Mental health and community partners in reviewing data to support Tier 2 mental and behavioral health interventions.

Weaknesses based on Survey... 3.1 Tier 3 is in place as measured by scores on the BAT and Tier 3 TFI. Action Plan… Review data from ISF survey and implement action plans/strategies to address areas for growth.

Weaknesses based on Survey... 3.3 School and community employed staff participate in all Tier 3 student conversation/teams. Action Plan… Establish communication between school and community employed staff when discussing Tier 3 students.
