2010-교육미디어-1014 Grave Accident Case in Manufacturing Sector Victim Crushed to Death between Hydraulic Pusher and Compressor during Compressor Clean-up

Overview of Accident When: Jul. 31, 2008 15:30 Ⅰ When: Jul. 31, 2008 15:30 Where: XX Cardboard Company located in Seo-gu, Incheon City Who and how: The victim was caught between the hydraulic pusher and compressor when he was cleaning up the scrap paper compressor. Why: The victim was crushed to death on site and his right arm, left wrist and neck were nearly severed.

Details of Accident Ⅱ The victim arrived at the factory at 6:40 as usual and at 7:00 began piling up the craft paper at craft paper machine #4 and cleaning. The victim was going on vacation from 16:00 that day to Aug. 3 and was rather excited. The victim finished work with machine #4 at 14:40 and began cleaning the scrap paper compressor, hoping to finish it by 16:00. In principle, the scrap paper compressor needed to be stopped before cleaning. However, it was impossible for the victim to wait until the machine was stopped for him to finish the work by 16:00. So he risked cleaning the machine while it was still operative.

Details of Accident Ⅱ At 15:30, a Thai worker found the victim dead behind the scrap paper compressor and informed the craft paper machine #3 forklift operator who was cleaning the forklift near the compressor. The accident was confirmed and operators at 119 and 112 were contacted. Assumed cause of accident While the compressor was still working, the victim cleaned the inside of the hydraulic pusher with his head and hand inside it and when the pusher began operating, the victim had no time to escape. The hydraulic pusher’s cycle lasts 3~10 minutes and the victim presumed that he had enough time to clean it before it would start. However, it started sooner than expected and he was crushed.

Cause of Accident Failure to stop the machine before clean-up Ⅲ Failure to stop the machine before clean-up The representative of a business should ensure that a machine is stopped before it is maintained, cleaned, supplied with oil, inspected, repaired or other similar work when doing these jobs while it is operative may harm the worker. However, the scrap paper compressor that the victim was cleaning was still operative. Failure to install a cover and interlocker at risk areas There was no interlocker that could stop the machine if the cover of the opening of the scrap compressor that had a risk of crushing a nearby person was opened.

Prevention of Accident Ⅳ Stop the machine to be cleaned If maintenance, cleaning, oil supply, inspection, repair or other similar work done on a machine is likely to harm the worker, such a machine should be stopped while performing such work. When the machine is stopped, it should be locked and the key secured or a signboard attached to prevent any other work on the machine. If it is possible that the machine could start unexpectedly by improper work operations, a work supervisor should be positioned to ensure safety.

Prevention of Accident Ⅳ Install a cover over a risk area and an interlocker Install a protective cover over the risk area where workers can be crushed and an interlocker that automatically blocks the power supply when the cover is removed. Install a protective cover over the opening between the hydraulic pusher frame and compressor body and an interlocker that blocks the power supply when the cover is opened for cleaning, repair or check-up.
