Download and print a PDF of the application on insurance website, or complete it entirely online (preferred.) All Employees Must Re-enroll in Health Insurance. Make a choice between Traditional or PPO. If you need to complete the paper application, it will be accepted by the agency payroll office until May 28 by 5:00 P.M. Agency payroll office will enter your information via IPOPS. Agency payroll offices will receive a limited number of applications so apply early.
