Care Services Pharmacist in the Community Angela Groome angela.groome@boots.co.uk Providing Advice to Care Homes Clinical Expertise Working as a care services pharmacist in West of Scotland , I lead for patient care in care homes inspiring and involving colleagues in community pharmacy to optimise their patient care. This advice can be delivered via The pharmacist advice visit template which covers regulatory and professional advice pertinent to care services Tailored visits to support a care home with a particular area e.g. covert medication administration Adhoc advice when required (readily accessible via the telephone ) Side effects of medications Times of administration Interactions Cautions and contra indications Covert administration Medication administration through a variety of enteral feeding tubes Palliative care Community pharmacy polypharmacy review Antipsychotics in dementia PRN protocols Homely remedies High risk medicines e.g. warfarin, lithium, insulin , controlled drugs Compliance issues Advice on administration of medicine when resident / patient leaves the home for a period of time Advice in response to Care Inspectorate requirements or recommendations following an inspection - Inform future practice / developments for care of patients in care settings Working in partnership with Royal Pharmaceutical Society to implement the recommendations from “Improving Pharmaceutical Care in Care Homes” report 2012 Transition to original pack supply from community pharmacy Support reduction of inappropriate antipsychotic prescribing for patients with dementia Prototype for national MAR (medicines administration record) Non Clinical Expertise Prescription journey including timely reorder Training of care home staff and community pharmacy colleagues Recording of medication administration – MAR chart completion Legal aspects of medication administration Safe storage and disposal of medication Support with auditing of medicines usage Advice in response to Care Inspectorate requirements or recommendations following an inspection Support to minimise waste External stakeholders Care inspectorate – due to specialised knowledge of the regulatory environment provide bespoke guidance to support and improve care settings in medicines management in Scotland Clinical Governance leads / managers within the care homes – providing advice for the optimal care of their patients/residents Royal Pharmaceutical Society – working to inform best practice to optimise patient care in the care settings Health Boards – working together with Health Board colleagues to inform and implement best practice across all health board areas Acknowledgements I would like to give my thanks to Fiona Maclean (NHSGG&C) who helped with the design of this poster template.
