SAFE WORK INSTRUCTIONS Title of subject: Hydraulic Equipment Retriever Last reviewed date : November 2015 SAFE WORK INSTRUCTIONS The following safe work instructions is NOT INTENDED to replace hands on training requirements of the code of federal regulations. The intent is to provide an overview of known potential safety hazards associated with equipment operation. Hands on training with an experienced operator on each specific model must be conducted. The trainee can not proceed onto production until they are determined ready to proceed by the supervisor of the training. Before any operation or maintenance procedure is performed with or on this equipment, it is important that personnel have a complete understanding of the location and functions of all controls and indicators, as well as, the location and use of all emergency shutdown controls and procedures. Steps for Initial SWI Training: Prepare – Review the SWI. Present – Quick Card at the machine with experienced operator. Tryout – Person runs the machine with supervision. Follow up – Competency points are verified by supervisor and 5000-23 is signed by the trainee/trainer to complete the training. SAFE WORK INSTRUCTIONS Title: Hydraulic Equipment Retriever SAFE WORK INSTRUCTIONS KEY POINTS SAFETY PRECAUTIONS Examination of Work Area: Examine roof and ribs for loose materials. Examine travel ways for obstructions and adverse conditions. Scale roof and ribs as needed. Do not travel under unsupported roof. Pre-ops: Examine cables, hyd. Hoses, connecting links and chains for cuts, breaks, stretched links, and other physical damage. Ensure that the equipment is free from accumulations of combustible materials. Test all levers proper operation. Make sure all applicable guards are in place and secured. Check fluid levels to insure required amounts are present. Check for fire fighting materials. All connecting devices, wire/synthetic ropes, chains, and hyd. cables must be of proper size and free of any physical damage. Replace any and all recognized. Maintain a work environment free of combustibles.(coal and oil) All covers and guards must be in place. Make sure fire fighting equipment/materials are present during operation. Pulling stuck equipment: Utilize effective communication with the personnel hooking/unhooking the equipment. Ensure that all personnel are in the clear prior to initiating the pull or initiating movement. Ensure that stab jacks remain secure into the roof. Always be aware of high pressure hydraulics and stored energy in the ropes and chains being utilized. Always relieve pressure and confirm relief after a pull is complete. Use only properly rated chains, ropes and connections. Utilize remote control devices or other methods to position people out of the line of fire. Only one person shall give directions relative to the pull All travel way that allows access to the affected area must be dangered off with signs and flashing red lights. Post individuals at these locations if necessary. Verify radios or other forms of communication are functioning properly. Stab jacks pressured against the roof change the overlying strata. Always be aware of changing roof conditions in this area. Continually monitor ropes, chains and hoses. Repair leaks as needed. Slack in chains and ropes indicate pressure is relieved. In the system.
