This service aims to address the physical health inequalities experienced by people with a Learning Disability, to raise awareness of these issues, reduce stigma and increase social inclusion
What do we do? Support all GP Practices in Hull regarding the Learning Disability annual health check Raise awareness regarding the annual health check with patient and staff groups Develop and promote tools to support best evidenced based practice. Support services in complying with the Accessible Information Standard Improving information for prisoners Raising awareness regarding employment
