Wellbeing Service-Learning Disabilities Primary Care Liaison

Introductions

This service aims to address the physical health inequalities experienced by people with a Learning Disability, to raise awareness of these issues, reduce stigma and increase social inclusion

What do we do? Support all GP Practices in Hull regarding the Learning Disability annual health check Raise awareness regarding the annual health check with patient and staff groups Develop and promote tools to support best evidenced based practice. Support services in complying with the Accessible Information Standard Improving information for prisoners Raising awareness regarding employment

Reasonable adjustments are a legal requirement, not a choice The Equality Act (2010) places a duty on all service providers to take steps or make reasonable adjustments to avoid putting a disabled person at a substantial disadvantage when compared to a person who is not The Accessible Information Standard Became Law in July 2016 and aims to ensure people who have a disability, impairment or sensory loss are provided with information that they can easily read or understand.
