Folstein Exam Be prepared with: A blank piece of paper A pen or pencil The Folstein Form

Folstein Exam “Tell me the date” The first question: Let the patient answer, then prompt the patient for missing items, like “what season is it?” Score 1 point for each correct answer.

Folstein Exam “Where are you?” The second question: Let the patient answer, then prompt the patient for missing items, like “what county are we in?” Score 1 point for each correct answer.

Wait 5 seconds, then ask the patient to repeat the 3 objects. Folstein Exam The third question: Say slowly and clearly, “Dog, tree, car” (or any 3 objects) Wait 5 seconds, then ask the patient to repeat the 3 objects. Score 1 point for each correct answer.

Folstein Exam Total up the Score: Add up the patient’s score. 23 or below is Abnormal Cognition This test is also used for Alzheimer patients. Interesting that patients in car accidents have the same mental problem as Alzheimer patients… Could Alzheimer’s Disease be related to a previous trauma?

In Summary… The Folstein Exam is filled out Day 2 initially, at re-exams & at final exam. There will be 3 to 6 of these when you are ready to write the narrative report. It is part of the analysis for permanent brain impairment. Often, patients who score 25-28 on Folstein will still have several 2’s and 3’s on the Rivermead. Send all your Rivermeads and Folsteins to the MD to get an impairment done by the MD.

Colossus will NOT give any points unless you get an Impairment Rating based on the AMA Guides to the Evaluation of Permanent Impairments.

You must work with attorneys who know how to submit a Colossus-friendly demand letter to the insurance company… if you want to be paid in full on your lien.
