CSI - History June 2006 Original $250,000 Grant from CHCS to OHIC - Contract with QPRI to manage October 2006 Original Stakeholder members convened - Kick off with Ed Wagner. Plenty of skepticism. November 2006 - July 2008 Stakeholders work together to develop CSI goals, and original contract terms. Project dies and is reborn several times. July 2008 Commonwealth awards Grant to Harvard SPH to carry out CSI Evaluation October 2008 Original CSI Contract Implemented in 5 sites: Second multi-payer pilot in country to kick off March 2009 Practices produce first quality reports.

CSI - History Spring 2009 Original 5 sites all receive NCQA Level 1 Recognition April 2010 CSI Expands to 8 new sites, including the South Cty. Hosp. Patient Centered Medical Community, and Mem. Hosp. Family Care Center May 2010 CSI Adopts New Mission: To Lead the Transformation of Primary Care in Rhode Island July 2010 CSI Receives $125,000 grant from RI Foundation to provide technical assistance to practices August 2010 CSI Submits Application for Medicare Advanced Primary Care Practice Demonstration - one of 8 States selected

CSI - History Fall 2010 Expansion sites Receive Level 1-3 PCMH Recognition; Original Sites Receive Level 3 Recognition. Expansion sites produce first Quality Reports January 2011 CSI Steering Committee merges with Beacon Community Program - First joint meeting held Spring 2011 CSI becomes a program of the Rhode Island Foundation April 1, 2011 (sort of) After 9 months of negotiation, New CSI Contract Implemented: Focuses on increased payment for expanded services and decreases in utilization June 2011 CSI/Beacon Celebrate Five Year Anniversary!
