Authorization §42-19-30 Authorizes Commission to assess fines and penalties for failure to file required reports $100

Authorization to Double Fines § 42-3-105 Authorized Commission to double fines and penalties – Full Commission voted to double fines beginning April 1, 2009 $200

Form 19 § 42-9-270 Failure to file Form 19 - Notice of Final Payment Fine $50

Review and Abatement R 67-1401 Request abatement in writing within 30 days Include reasons for contesting assessment File with department issuing assessment Department director will review May abate for good cause Determination set forth in writing Party may request Order and Rule to Show Cause hearing

Informal Conference R 67-804 (H) Fine assessed for not appearing at an Informal Conference $188.36

Form 12-A R 67-411 Due within ten(10) business days of the employer’s representatives knowledge of the accident If accident meets the criteria to file with the Commission

Form 15 R 67-503 & R 67-504 Section I Filed within ten(10) from the date of the first payment of compensation Section II Filed immediately to give notice compensation has been stopped

Form 17 R 67-505 Filed with the Claims Department Within thirty-one (31) days of the date the claimant returns to work or Has agreed he/she is able to return to work

Form 18 R 67-413 Due six (6) months from date of the accident and every six months thereafter until the file is closed with the Commission

Form 19 §42-9-70 Filed by employer’s representative Due to the Commission within (16) days of the final payment of compensation

Form 19 R 67-414 Required to close all files even if no compensation has been paid

Form 20 R 67-1603(D)1 Employer’s representative requests informal conference or hearing

Form 20 R67-1603(D)2 Claimant requests an informal conference
