Due March 1st 2016! FMRSP Final Report ***You must have written permission to submit any later*** Email completed form to FMRSP@bruyere.org - cc your FMRSP Supervisor upon submission! Signed agreement form no longer needed December 2015

Final Report - Late Submissions Your Final Report will not be considered for any RIO day awards Unit Director, Unit Coordinator, Post Grad Director and Post Grad Committee will be notified November 2015

FMRSP Presentation – RIO Day Thurs June 9th 2016 ALL DAY EVENT followed by graduation! 10 minutes presentation + 3-5 minutes questions / discussion December 2015

To be eligible you must submit your report by March 1 2016 RIO Day awards To be eligible you must submit your report by March 1 2016 Top 10 projects will be announced during RIO day Prizes will be awarded for the top two FMRSP reports, and top QI project Prizes will be given for best presentation in each room (4-5 rooms) December 2015

FMRSP – Best Project Supervisors Award Email will be sent out in April requesting nominations for Best FMRSP Supervisor Submit text (200 words max) and name of supervisor to be nominated to FMRSP@bruyere.org outlining how the nominee showcased outstanding and sustained support within your FMRSP Project. Deadline May 9th 2016 November 2015
