Annual Enrollment November 1 – 17th, 2017 Annual Enrollment begins on November 1st and ends on November 17th midnight Central Standard Time. As a benefits eligible employee, annual enrollment is your once chance each year to make changes to your benefit coverage for yourself and your eligible dependents.

Qualified Status Change Event is not required during Annual Enrollment

Changes Take Effect January 1, 2018 Evidence of Insurability (EOI) changes may take effect later depending upon approval. Changes made during annual enrollment will take effect January 1st, 2018. Certain insurance plans that may require Evidence of Insurability or EOI, changes may take effect later depending on approval.

Health Savings Account HSA – Benefit Wallet Can only enroll if currently enrolled in a HDHP (BlueEdge) Cannot have a HSA and a FSA in the same calendar year OPSU will contribute up to $1,000 into HSA for employee only and those with dependents up to $1,500 annually 2018 limits Individuals - $3,450 Family - $6,900 Catch-up $1,000 annually for age 55 or older

Additional Changes Employees can now cover their OPSU spouse on the health plan. In most cases, this will not be financially beneficial No double coverage on the health plan; i.e. enrollment in the OPSU Employee Only plan AND covered on the OPSU spouses plan too

Life/Accidental Death & Dismemberment and Long Term Disability Changes

Life/AD&D and Long Term Disability LTD and Supplemental Life Products 0% increase three year rate guarantee Liberty Mutual - Long Term Disability 60% up to $6,000 a month benefit

Life/AD&D and Long Term Disability Supplemental life Employee – GI – 2x salary up to $300,000 max, $10,000 increments. With Evidence of Insurability (EOI): 5x salary, up to $750,000 Spouse – GI – 1x employee salary up to $130,000 max, $10,000 increments. With EOI: up to $380,000 Child(ren) – $2500 increments up to $10,000 Open Enrollment Employee – may increase by 4 increments up to GI Spouse – may increase by 1 increment up to GI During annual enrollment, Liberty Mutual will offer the guaranteed issue without EOI, even if you were previously turned down for coverage. Also, the employee supplemental life insurance guaranteed issue will increase to 2 times annual salary up to $300,000. Spouse supplemental life insurance will now be 1 times employee salary up to $130,000 guaranteed issue up to $380,000 with EOI in $10,000 increments. There will be no changes to the child life insurance increments. There are no changes to any of the supplemental life insurance premiums.

Make sure you elect “Save & Continue to proceed to the next page.

This is a pop up message from the Benefits Team to serve as guidance throughout the process. After you read it, select “Close”.

Your elections will be displayed Your elections will be displayed. Should you want to change the coverage in which you’re enrolled, select “Edit Coverage”.

Until you are all the way through the elections, to either enroll or decline, you can select the green button but it will say “Take me to the benefits left to complete”. At the very end of the review, you can then select “Complete Enrollment” for a confirmation.

The rates shown will be the 2018 rates; not the 2017. If you are currently enrolled but want to decline, you can select “Edit plan” and you will be able to decline the coverage.

Don’t forget to “Save” at the end of every group of benefits in order to proceed to the next group.

Select “Begin Enrollment” to proceed through the benefit groups.

Once you have made all the changes you want to make, select “Complete Enrollment”.
