Current Model Last underwent open tender in conjunction with Rotherham in 2013 Current provider is South West Yorkshire Partnership Foundation Trust Incentivisation of quit rates for face to face and non face to face work A 2 tier quality premium for 20.0% total of contract value Must earn quality premium 1 before 2 can be earned 16 measures to be met A relatively complex structure

New model from 1 April 2018: key features A Doncaster specific model Individualised care, rather than face to face or non face to face pathways- including psychosocial support for e-cigarette users Targeted priority groups A somewhat simplified performance related payment based on ‘routine and manual’ validated quits (10% of contract value)

New model- the targeted groups: Routine and manual workers Mental health clients Prisoners on release Chronic disease/complex health needs/pre/post operative Reducing inequalities between communities Targeting these groups will require the development of pathways into the service

Exclusion criteria Children and young people (under 18) Pregnancy/and significant others during pregnancy and 1 year post natal Hospital inpatients/prisoners in custody

Key outcomes/contract aims To reduce prevalence of smoking in targeted groups in Doncaster To contribute to a reduction in smoking related mortality in Doncaster

The Contract 4 years plus 2 12 month extensions possible Opportunities to subcontract From £649,420 to £619,160 per year taking account of 2.5% efficiency savings for service costs built into years 2, 3 and 4 £2,536,650 over 4 years 10% of annual service costs linked to validated 4 week quits (530 per annum, taken from 15/16 baseline performance) of routine and manual workers
