Problem / Risk Indentified Patients suicidal cases found inside shower rooms. II. Objectives To modify the toilet fixtures to prevent patients from committing suicide. III. Action taken During May to August 2008, R&M team had replaced: shower room panel and door with curtain and plastic rail. shower head holders with plastic ones. metal hooks with plastic ones. curtain and plastic rail Plastic hooks IV. Results improve patient safety
