MCP Voluntary Contract

The MCP – What is it? A population based care model aiming to improve the physical, mental and social health wellbeing of a local population. Based on the General Practice registered list. New model of enhanced primary and community care.

MCP Care Model Natural community of care (neighbourhood) – 30-50,000 Services delivered through “care hub” which has a multi disciplinary team supported by GPs Natural communities combine to a population foot print of 100,000 plus

The Four Levels of MCP Care Model

Contract is Voluntary . . . there are 3 options Virtual MCP Partially Integrated Model Fully Integrated Model

Virtual MCP Practice commissioned under GMS, PMS or APMS Practices sign an “alliance agreement” with commissioners and other providers Sits on top of contracts Alliance Contract NHSE CCG LA GMS PMS APMS NHS Standard Contract Other Contracts Community Services Social Care GPs Mental Health

Partially Integrated MCP Commissioners award a contract for all services in scope – except core general practice. Practice keeps GMS or PMS contract but signs an “integration” agreement with MCP provider. This is a local agreement Existing GMS/PMS/APMS GPs GPs Community and MH services Some hospital services Social Care MCP Contract GPs GPs GPs

Fully Integrated MCP MCP Contract Commissioners award a contract for a full range of services for population which includes core general practice. GPs able to give up or suspend GMS or PMS contract – become MCP owners employees. Primary care funding becomes part of the MCP whole population budget. GPs GPs Community and MH Services Some hospital services Social Care Primary care GPs GPs

The Contract Sum Capitated budget covering all services within the contract – provider deploys flexibly. MCP pay for performance – QoF and CQUIN. Gain risk share arrangement.

What could be included in the Contract? Community Nursing and Therapy Some Hospital Services (Diabetes) Diagnostics Sexual Health Mental Health Ambulatory Care

What benefits could there be to GPs? Greater influence and control over the local health and care system including control over the allocation of financial and human resources in the community. Work in a care model built by GPs that allows GPs to have a work/life balance which will in turn increase recruitment and retention. Make helping those with complex problems easier by offering a greater access to a broader, more in-depth range of services in primary care settings. True multidisciplinary working that reduces hand offs to general practice from other clinicians and professionals. Wider career opportunities for GPs that allow portfolio careers.
