My Big Heart: Managing Heart Failure at UCLA Code for the Mission My Big Heart: Managing Heart Failure at UCLA

Problem: Heart failure is costly for UCLA Heart failure is prevalent. 5.8 million people in the Unites States suffer from heart failure Annual health care costs are estimated at $30 billion A majority of these cost are due to frequent hospitalizations Predictors of heart failure and re-admissions has been studied. There are multiple parameters that predict heart failure exacerbations

Problem: Do our patients understand? Poor understanding of their disease Poor compliance with medication and diet recommendations Missed or lack of follow up appointments

Mobile App – What will we aim to do? Educate patients Track real-time data on patient heart rate, blood pressure, pulse oximetry, and weight through blue tooth monitors, or wearable devices Survey patients on set of clinical symptoms

Mobile App – What will we aim to do? Monitor medication compliance Design an automated notification system based on an algorithm aimed at identifying the threshold of heart failure exacerbation Interface with care connect

App Features MDs through care connect can send medications and tasks to the app for patients to track Data can be reviewed by NP or MD at clinic visits to monitor compliance Alternatively data can be sent back to care connect Carvedilol 25mg Furosemide 40mg

App Features MDs can send specific daily or weekly survey questions to patients Develop an algorithm to identify heart failure exacerbation and notify patients to call MD. Notify patient about critical vital signs Translate to other departments Respirations Shortness of breath Chest pain

Proof of concept/ Minimal viable product

Submission requirements The technology product submitted will consist of a mobile native or web app. A link to an emulator or video of the app’s capabilities and functionality must be entered into the “Website of app created” field on the Technology Product Submission Form. A PowerPoint presentation no longer than 10 slides or a Word document no longer than 5 pages describing the web or mobile app must be uploaded to the Technology Product Submission Form and accompany the submitted entry. The following must be submitted via the submission form (not yet available) by 08/31/16
