Multi-procedure Synopsis Data Intra-procedure Synopsis Data SYNOPSIS IN THE ELECTRONIC MEDICAL RECORD: RECAPTURING THE APHERESIS PATIENT STORY Tomas Armendariz BSN, RN, CMSRN, Shelli Chernesky MBA, MSN, RN,CCRN, Christina Lin BS, EMR Analyst, Nicole De Simone MD, Ravi Sarode MD Apheresis Center, Department of Pathology, University of Texas Southwestern, Dallas, Texas Purpose Apheresis Synopsis Multi-procedure Synopsis Data Intra-procedure Synopsis Data To report the implementation of a concise and easily accessible “apheresis synopsis” report to improve team communication, overall efficiency, patient safety and comfort. Background With the birth of electronic medical records (EMR), the patient story has been lost. In our EMR (Epic®), physicians do not have access to apheresis display screens due to differing configurations based on provider type. Procedural data, such as inlet flow rates, use of calcium to treat citrate reactions, or use of tissue plasminogen activator drives future treatments. EMR user inefficiently toggled between different tabs of the chart: Labs Vital signs Physician/APP notes Nursing/Technician notes The synopsis captures: Type of procedure, operator name and pertinent labs Interval intra-procedure vitals signs, flow rates, type of replacement fluid, adverse reactions and treatment Post-procedure parameters from the apheresis system Implementation Conclusion Collaborative development of custom reports by apheresis nurses, physicians and EMR application support teams The synopsis screen provides an easy to use tool to revisit prior procedures and adjust parameters for subsequent treatments. Communication between nurses and physicians has improved leading to personalized and safer patient care. This material contains confidential and copyrighted information of Epic Systems Corporation. Further inefficiencies occurred due to having to toggle between tabs in the EMR, such as between prior procedures notes, labs and recent vital signs to determine parameters for the day’s procedure.
