COPD Case Summaries: Natalie Woronchuk RN, BS, BSN – Physician’s Health Alliance JM – 78 yo male LJ – 75 yo female PMH: COPD, CAD (MI, PTCA), HTN 1st admit 10/2010 - CM transition call 2nd admit 12/2010 -CM transition call and initiated Care Plan and CM f/u q 2 weeks x 6 weeks - monthly 3rd admit 3/2011 – exac COPD: difficulty returning to baseline; DME 4th admit 4/2011 – exac COPD: decline in status; weight loss HH f/u with pulmonary rehab; breathing exercises; pulse ox with exercise; meal supplements, pulmonologist standing orders Doing well, weight increase, returning to baseline and usual activities PMH: COPD, CAD, angina, HTN 2/2011 Referred to CM by PCP – declined follow up 1st admit 3/2011 – resp distress; anxiety- CM transition call and pt now agreeable: initiated Care Plan and CM f/u 2nd admit 5/2011 – resp distress; HH for extended time – returned to baseline ED visits 7/2011, 9/2011, and 10/2011 for breathing “worked myself up into a tizzy”
