INSTRUCTIONS FOR COMPLETION OF CCF 104-R

THE COMMON SENSE BLOCKS IF YOU MESS THESE UP YOU PROBABLY SHOULD CHOOSE ANOTHER CAREER PATH… BLOCK 1: NAME BLOCK 2: ACADEMIC MAJOR: What it currently is or what you believe you will major in (REMEMBER… after contracting you must receive permission from PMS to change major and you are only permitted to change your major once) BLOCK 2a: Leave Blank BLOCK 3: DATE prepared BLOCK 4: THE SCHOOL YOU ATTEND a. PRINCETON = HOST, ROWAN & TCNJ: EXTENSION CENTER, RIDER & RC: CROSS ENROLLED b. PRINCETON UNIVERSITY (for all schools) c. 002627

THE MOST IMPORTANT… AND DIFFICULT… BLOCK TOTAL REQUIRED FOR DEGREE: Get out a calculator and add up all of the credits you have and will take that are listed within the boxes in BLOCK 7. ROTC hours that do not count = 0 CREDITS TOWARD DEGREE COMPLETE: Add up all of the credits listed in BLOCK 7 that you have already completed. Do not include transfer credits. TRANSFER CREDITS ACCEPTED: Write the number of transfer credits that have been accepted by your university THAT COUNT toward the completion of your degree. If you are not a transfer = 0 D & E: These will self-calculate. It is very important that the number in ‘e.’ depicts the exact number of semesters you have left to complete, including the current semester. If you are a freshman in your first semester, this number should calculate to 8. If this block DOES NOT depict the number of semesters you have left, contact Ms. Cvet

We want these numbers above 3.0! If you aren’t there, get there! BLOCK 6 We want these numbers above 3.0! If you aren’t there, get there! Complete your GPA data for each of the semesters you have completed to date. Term = Fall/Spring/Summer/Winter ‘XX (year). Ex: Fall ‘15

NOW FOR THE FUN PART All classes that you have and will take at your university need to be listed by semester. If you are a transfer, include the courses that your university accepted for credit. Each block needs to reflect a single semester. Note: Must include military science courses: lab and class (ex: 101, 102, 201, 202…). If you do not receive credit for ROTC, fill in 0 (zero) for the credits. Term: Spring/Fall/Winter/Summer Year: two digit, ex: ’15 No.: Course code listed on transcript Course Title: Name of course Hours and Credits: Should be the same. PRINCETON STUDENTS: We count each Princeton course as 4 credits but military science should still reflect 0. Grade: The letter grade received. If you have not completed the course yet, leave the grade block blank. The final block should be your last semester on campus and informs us of when you will be graduating. NOTE: YOU MUST TAKE A MILITARY HISTORY COURSE AT SOME POINT. YOUR 104R SHOULD REFLECT THIS COURSE IN ONE BOX. FORMS WILL NOT BE ACCEPTED W/O IT.

BLOCKS 8 - SIGNATURES BLOCK 8: Do not date or initial yet. When you are counseled by your instructor you will complete this box BLOCK 9: Check ‘YES’ and write the degree you intend to earn as well as the month and year you will graduate BLOCK 10-13: This is where your academic advisor should sign the document after he/she has reviewed its entirety. Don’t forget to sign and date yourself before turning in to cadre. LAST PAGE: Fill in the three boxes within the paragraph. Input the university that you academically attend. SIGN AND DATE AGAIN, and then turn in to cadre for PMS approval and signature.
