Mission Statement The mission of Divinity Transportation, LLC is to provide our shippers reliable shipping services in a prompt and timely manner one truckload at a time.

Performance Evaluations Evaluations are conducted to provide both supervisors and employees the opportunity to discuss job tasks, identify and correct weaknesses, encourage and recognize strengths, and discuss positive approaches for meeting company goals.

Pay Day Things to Remember Settlements are deposited in your payroll account every other Friday All drivers must scan in a copy of their bills once they deliver using the Scan and Go at any Flying J. Company code: 892900000 Original Bills, fuel receipts, and log sheets must be submitted for payment which can be submitted using our fedex billable stamps (cost $16.50 taken from settlement) Bills are due by the Monday before a payday at 5:00 pm If payday falls on a holiday, employees will receive pay on the last work day before the regularly scheduled payday. We are compensated on a bi-weekly schedule (every other Friday). In order for you to be compensated, you must submit your original bills, fuel receipts, and log sheets the Monday before the payday by 5 pm. If your paperwork arrives after this cutoff you will not be paid for that paperwork submission until the following scheduled payday. A payroll calendar can be downloaded from our website under the employees only tab. To ensure our over the road drivers have ample tim e to submit their paperwork, you are given Fedex billable stamps to submit your paperwork. A $16.50 charge will be deducted from your settlement for each stamp used,, so we recommend you send only packet per week for all work performed. Over the road drivers are also required to scan their bills after they deliver their load to dispatch before they will be reloaded using the Scan an GO at any flying J location. Our company code for the scan and go is 892900000. Payroll text messages all over the road drivers a reminder the Wednesday before the due date to give them a 5 day notice of the due date. If a payday falls on a holiday, you will be paid the day before the holiday. Over the road dirvers are eligible for advances up to $300 per week if money is owed to them. In your orientation packet ther e is a fleet payroll card application. Make sure you complete this form. Our drivers are paid on their payroll mastercard to ensure they can access their money on payday. That form will be faxed to Transportation Alliance bank. A bank representative will contact you to verify security questions. After this contact, they will mail you your account number and card. As soon as you get this information in the mail, contact payroll via email at payroll@divinitytransportation.com and send them your account number so that you can be added to our payroll system. If we do not have your account number in our system a paper check will be mailed to your home address. Your debit mastercard can be used anywhere mastercard is accepted, such as Wal-Mart, local restaurants, atms, etc…. Things to Remember Advances will be deducted from bi-weekly settlement

If you decide to work on a Holiday, you will receive extra pay. Holidays 8 unpaid holidays as listed below: New Years Day (January 1) Martin Luther King Jr. Day (3rd Monday in January) Independence Day (July 4) Thanksgiving day (4th Thursday in November) Day after Thanksgiving (4th Friday in November) Christmas Eve (December 24) Christmas day (December 25) Day after Christmas (December 26) If you decide to work on a Holiday, you will receive extra pay.

Time Off Sick Leave (7 days/yr) Vacation Leave (accrual) Military Leave (varies) Maternity (6-12 weeks) Paternity Leave (1 week) Bereavement Leave (4 days) The reasons for time off listed on this slide are intended for internal staff members. Owner Operators are given the benefit of adjusting their own workschedule as needed for any of these reasons listed on this slide, however during your 45 day probationary period you are expected to work your regular work schedule the entire time so that we have the opportunity to fully access your work performance. If an emergency presents itself, simply notify dispatch immediately so that your work schedule can be adjusted. Keep in mind that you will be required to provide us with documentation concerning your absense before you are allowed to return to an on-duty status. Time Off

Policies & Procedures Workers Compensation Drug & Alcohol Use Sexual & Other Unlawful Harassment Employee Conduct Work Rules and Separation Procedures Separation As independent contractors you are not eligible for workers compensation. You can purchase accidental occupational insurance that works the same as workers compensation. If you are hurt on the job the insurance will pay your medical claims and provide you with a check to pay your personal monthly expenses. If you would like more information about this insurance, contact human resources via email at hr@divinitytransportation.com Drug and alcohol use is strictly prohibited here at Divinity Transportation. All over the road drivers will have to pass a drug screen before permanent employment can be extended to them. In addition to your initial screen, all drivers are entered into a drug and alcohol consortium and will randomly be drug screened by a Dr in your area. If you are in an accident, you must be drug screened within 24 hours of the accident. If you refuse to take a drug/alcohol test or if you fail one, you will automatically be seperated from our company. There are a number of reasons why you can be written up and seperated from our company. Profanity towards staff members, not reporting a failed DOT audit, stealing from the load, lying about your location, and a host of other reasons. Before we separate you (terminate your employment) we require that you do an online exit interview and return all company issued property. You will not be paid your final settlement until all property has been returned. If you decide to terminate your employment with us, we ask that you give us a 2 week written notice. If you fail to do so, you will be fined $500 per week that notice was not given. This assists recruiting with screening and processing a suitable replacement for your position.

Use of Company Vehicles Rules and regulations

Communication is Key! When should I contact dispatch? Remember drivers must notify Dispatch immediately upon pick-up and drop-off for all loads (including brokered freight). Failure to do so will result in a $100 fine for each occurrence and disciplinary action up to separation When a load is picked up When a load is delivered For scheduled maintenance needs At the start of my shift For directions For all work related questions When should I contact dispatch?

Log Sheets Below are a number of rules to remember:   All drivers must have a 10 hour shut down daily (sleeper) with 8 consecutive hours Drivers cannot drive longer than 11 hours per day All fuel stops must be noted on log sheets Origin and destination must be noted on all log sheets Off-duty time includes time when you are on your 34 hour reset Drivers can only operate for 14 consecutive hours (on-duty not driving + driving) before they have to shut down Your 14 hour clock begins when you leave the sleeper A vehicle inspection must take place daily before you begin your shift Company speed Limit is 67 MPH- 3or more speeding violations on your log sheet audits will result in $45 fine
