HSE Performance 2012-2016 1100 In Dec 2015, we launched the NEAR MISS REPORTING TOOL and dramatically increased near miss reports by >100% per month. In previous years, there were not enough near misses being reported, hence, we failed to create a learning environment and prevent tragic incidents in PDO. The ratio of near miss to LTI or TRC does not satisfy as per the Heinrich safety pyramid. 251 246 231 191 187 181 167 179 154

Near Miss Reports 2015-2016 441 A consistent improvement on near miss reporting across ALL Directorates. Averaging 40 reports per week in 2016. Top HSE Categories Reported - NM 2016 It shows the main focus areas which have to be addressed at site. It shows what can happen when weak signals were not reported immediately. There is a continuous drive to report near misses in both interior and coastal.

Hazard Hunt – Top9 Exposures The Hazard Hunt will cover activities on the sites visited. The Hazard Hunt team will also include reference to the identified top9 HSE exposures . The Hazard Hunt team will relate to Life Saving Rules where appropriate.
