CRUSHED FINGERS ! LTI # 01 01/Jan/02 The task: unload this rock-breaker and the steel bucket from the low-bed trailer. SEQUENCE OF EVENTS On Sunday 04/03/01 at 1800Hrs Maqbool and Faisal decided to leave WPH13 camp in Yibal, without permission, and go home to Muscat for the EID celebrations. They secured a lift from one of their colleagues to Nizwa and continued to Muscat by taxi. Their aim was to pick up a car (Jeep Cherokee) belonging to another colleague and return to Yibal in time for work starting at 1300Hrs on Monday 05/03/01. They left Muscat at approximately 0930 Hrs and at 1130Hrs were on the Nizwa-Izz section of the highway when they sustained a blow-out to the offside rear wheel of the vehicle. The vehicle went off the road on the right and into the ditch, they slid for approximately 40 metres then somersaulted four times coming to rest 146 metres from entering the ditch. Both pronounced dead at scene. 1

(Not able to make 1300Hrs start time) 4 Disregard for Omani Law (Speed Limit, Wearing of Seat Belt) 5 Failure to recognise hazards 2

LTI # 01 What happened ? 01/Jan/02 The trailer driver and rock-breaker operator were lifting the bucket with the rock-breaker. The driver was to hook up the sling. His hand was on the bucket. The chisel head jerked forward, and trapped the driver’s hand breaking 2 of his fingers. He will be off work for 2-3 weeks. IMMEDIATE CAUSE(s) 1 High speed, from investigation speed was in excess of 150KPH. 2 Failure to wear seat belts. UNDERLYING CAUSE(s) 1 Unauthorised trip. 2 Urge to be with family during Eid celebrations. 3 Late in leaving Muscat. (Not able to make 1300Hrs start time) 4 Disregard for Omani Law (Speed Limit, Wearing of Seat Belt) 5 Failure to recognise hazards Ouch!! 3

LTI # 01 Crushed Fingers 01/Jan/02 What caused this LTI ??: - Position: hand placed where it should not have been! - Hazard: not seen, not identified. - Communication: driver and operator did not understand each other and had not discussed the method before starting the work (tool-box talk). - Rushing: tried to get the job done too fast. Discuss in your team: - How could this incident have been prevented? - Do you have toolbox talks on “simple” jobs?
