Tier 3: Individualized Intervention Tier 3: Intervention Referral Meet to discuss targeted supports for student, complete “RtI Tier 3 Parent Notification” & “Tier 3 Referral” forms Teacher & BLT leader will review the Tier 2 Gen. Ed. Support plan in Mileposts & create a new Tier 3 Milepost Plan. BLT schedules meetings for Tier 3 intervention supports BLT & team determines function of need for intervention planning Student receives intervention from gen. ed. teacher as determined by the RTI team Student progress monitored every 2 weeks by general ed. teacher BLT review data weekly for ongoing decision-making. Repeat steps 4-6 in the “Mileposts Plan” for weekly, ongoing decision-making. 1/24/17
