Explanation Peak flow would be investigation of choice for asthma. An echocardiogram would be used to diagnose Valvular disease. An ECG may be used to show ischemic changes in MI or heart failure. It can also be considered in COPD if features of heart failure or cor pulmonale are present. Sputum culture would be used to diagnose a pneumonia. May also be considered in COPD to identify organisms if recurrent infection. COPD should be suspected in anyone aged over 35, with any risk factors (eg smoking) with: exertional dyspnoea chronic cough (3 months +) regular sputum production regular winter bronchitis wheeze A diagnosis of COPD is made using spirometry, where the FEV/FVC ratio is < 0.7. FEV1 value can also be used as a good indicator of future morbidity and mortality. A chest X-ray is also suggested on diagnosis in order to rule out a tumour or pneumonia, but cannot be used to diagnose COPD.
