Learning and Action Network Partnering to improve patient experience at the point of care and engagement of patients and families in policy and procedure making and in practice governance Collaborating with experts from the TCPi Support and Alignment Network (SANs):

Collaboration Partnering with the Stakeholder Engagement and Practice Transformation Teams at the Mid-South Practice Transformation Network to invite their ideas and experience. MSPTN involves practices aligned with the Vanderbilt Health Affiliated Network, Baptist Memorial Health Care (Memphis), the Safety Net Consortium of Middle Tennessee, and the Mississippi Affiliated Health Network throughout Tennessee, and in Mississippi, Arkansas and Kentucky. Learning and Action Network The Learning and Action Network will assist practices in improving patient experiences and satisfaction These components are included in the Merit-Based Incentive Payment Systems (MIPS) and Alternative Payment Models (APMs) Higher levels of patient satisfaction are also correlated with improved quality and provider profit margins The PFE measures will also be added to the Practice Assessment Tool (PAT) by summer 2017 Vizient Presentation │ Date │ Confidential Information
