Readmissions Bundle Elements Work in progress follow-up phone calls in 72 hours PDSA/Pilot work Physician Notification of Readmission daily report of 30 day readmissions automatically populated forward to the lead hospitalist (biggest volume) Still working on closing the loop

Readmission Data Collection Methods Population- all Trigger Tools- Daily alerts of readmissions triggered off of days since last admission Chart Review Tools- none

Readmission Interventions

Readmission Rate Current rate (rolling 12 months) Exclusions 7day- 2.6 30day- 6.2 Exclusions neonates(<30days) do the neonates as a separate report for the Neonatologist and Newborn Pediatricians Planned readmission (Chemo, ect)

Readmission Best Practice Recommendations We have done discharge phone calls for years, but the 72 hour turn around has made huge difference
