Elementary Documentation for Response to Intervention Presenter: Brandi Maynor, 2nd grade teacher at Blacksburg Primary School

This session will give primary and elementary teachers different ideas for writing and organizing anecdotal notes.

Anecdotal notes are used when observing students’ work, skills, and behavior. These provide information on student learning and direction for further instruction, along with important data to use at RTI meeting and parent conferences.

Anecdotal notes are Findings written down by the teacher during observations during independent work. We all conference during independent reading and writing time but writing it all down is the difficult part. I will begin conferencing Students the first week of school!

These are examples of how I keep track of my anecdotal notes for independent Writing and reading Work. I use 2 X 4 avery Shipping labels. I use Sheets of labels each Week for my Anecdotal notes. I keep anecdotal notes on each child weekly for each subject.

Once I have completed my Next Step Guided Reading Assessments and STAR testing, I will be able to identify students who need extra interventions. (Tier 2 and Tier 3)
