Case presented by Dr Schwartz 77 yo man: PMH – multiple medical problems 2003: L hemicolectomy stage III colon CA 6 mos adjuvant 5-FU/LV 8/2007: orchiectomy for DLBCL R-CVP x 8 (h/o valvular heart ds), persistent tx-induced neuropathy 8/2009: NED 2010: abdominal pain CEA >1200 PET/CT = liver lesions and 6.8 cm RP mass (? etiology) Biopsy x 2 = met colon CA
Case continued: 10/2010: FOLFIRI but enteritis and GI bleed post cycle 2 despite dose reduction 11/2010: Kras status identified from liver bx = wt Irinotecan/cetuximab with manageable rash and diarrhea CEA falling rescan pending
