Eat 4 Health Tier 2 community based weight management and Preventing Diabetes programme "If you're tipping over into diabetes, you're better off with diet and exercise than you are with medication." -Francis S. Collins, 2010 Research shows that modest weight loss and regular physical activity can help prevent or delay type 2 diabetes by up to 58% in people with pre-diabetes. Getting at least 150 minutes each week of physical activity, such as brisk walking, also is important. E4H can help participants adopt the healthy habits needed to prevent type 2 diabetes along with empowering them to lose weight and maintain it by leading a healthier lifestyle. Point 4: Ability to adapt its weight management approach to meet the needs and preferences of target groups Point 5 explanation: providing the knowledge of the relationship between healthy eating, physical activity, weight loss and maintenance as well as the skills to achieve this healthy lifestyle

Programme Outline A multicomponent 10 week programme based on MI technique delivered in group settings by qualified E4H Instructors 45 minutes of interactive nutrition workshops including portion sizes, fats & sugars, reading labels etc. 45 minutes of tailored exercise sessions towards the client’s ability Sessions structured on the evidence-based E4H manual Follow ups at 6 and 12 months by the E4H team Variety of measurements taken at week 1 and 10 to identify changes in fitness and weight Trained Eat 4 Health Instructors lead classes to help participants improve their food choices, increase physical activity, and learn coping skills to maintain weight loss and healthy lifestyle changes. Self-Monitoring- Pedometers. Food diary, regular weigh-ins

Referral Options To be referred, patients must fit the following criteria: BMI over 25 kg/m2 Aged 16 years or above Resident or registered with a GP practice within Reading, West Berkshire or Wokingham Self-referral forms widely available and can be picked up at venues across the localities Healthcare Professionals provided with Referral Packs including Professional Referral Forms, EMIS, vision templates Exercise clearance is provided by healthcare professionals for clients with contraindications. Many referral transfer options including post, email, web based forms online through E4H website or simply calling E4H team. An easy, straightforward referral process for self-referrals and healthcare professionals partnerships/training for a range of referring partners on raising the issue of weight point: Weight, height, blood pressure, knowledge of diet, levels of physical activity- with the aim to lose 5% of body weight by week 10

Course Locations We work closely with community groups and local to identify locations suitable for the delivery of Eat 4 Health courses Local venues including leisure centres, gyms, community centres, village halls, etc. Easily accessible, familiar to clients and parking or public transport links available Delivered on days and at times convenient for all including evenings and weekends
