COMMUNITY PARTNERS PROGRAMME CPP Federal Government Programme FOR ELDERLY PEOPLE FROM ETHNIC COMMUNITIES

THE COMMUNITY PARTNERS PROGRAMME (CPP) As we get older we may choose to get support in the home or in the community. Later we may want to live in an aged care facility. Organisations that provide support to the elderly are called “aged care support service providers”.

WHAT DOES THE CPP WORKER DO? The CPP worker is the link between the aged care services and the German community We tell aged care services about the German community And we tell the German community about aged care services Aged Care Service Providers CPP Nelke and Julia German community

WHY? SO THAT ELDERLY GERMAN PEOPLE HAVE GREATER ACCESS TO CULTURALLY APPROPRIATE AGED CARE SUPPORT SERVICES HOW: The CPP worker gives information to the German community on aged care services so you can make the right decisions AND The CPP worker teaches Aged care services about the German community and their needs so they understand you better

WHAT IS CULTURALLY APPROPRIATE CARE? Culturally appropriate care aims to provide what is important to you! Culturally appropriate meals

WHAT IS CULTURALLY APPROPRIATE CARE? Recognises German cultural celebrations and supports activities and festivities throughout the year

- Welcomes the local German community and families WHAT IS CULTURALLY APPROPRIATE CARE? - Welcomes the local German community and families visiting residents - Helps display cultural artifacts - Provides bi lingual or bi cultural workers - Provides literature in German - Uses interpreters

- Meet with Aged Care Support Services in the Illawarra More aged care support services delivering culturally appropriate care to older German people HOW? CPP workers to: - Meet with Aged Care Support Services in the Illawarra - Provide German cultural information kits - Teach Aged Care Services about German culture and the needs of elderly German people

CULTURAL LESSONS The CPP will teach aged care support service providers about your community - Culture - Traditions - Family life

INFORMATION MEETINGS THE CPP PROJECT Teach elderly Germans about aged care support services so they are able to choose what they want. SUPPORT IN THE HOME: THE CPP PROJECT

INFORMATION MEETINGS - Once every quarter - Guest speaker from Aged Care Service provider will visit - Interpreter (if needed) - Information in your language and English

HOW YOU WILL BE ABLE TO MAKE THE RIGHT DECISION - You will learn about the types of aged care support services available - How much they cost - Your rights

HOW WILL YOU KNOW ABOUT THESE INFORMATION MEETINGS - Advertising through a two page newsletter - Advertising through your ethnic newspaper - Telling each other about it - We would like your carers and family members at the meetings - 2 Vox FM 106.9 German Radio Programme
