Health Promotion and Preventive Services Review of MIPPA Grant Focus #3 April 12, 2017

MIPPA 2014 Grant Provide outreach to Medicare beneficiaries with particular focus on Identifying people with limited incomes who are likely eligible for Low Income Subsidies (LIS) and Medicare Savings programs (MSPs) Providing general Medicare Part D outreach and assistance in rural areas Promoting activities aimed at preventing disease and promoting wellness. Today we are going to review focus area 3 – Preventing Disease and Promoting Wellness

Health Promotion (HP) Programs Part of Older Americans Act (OAA) – Title IIID Must be over 60 to qualify for programs funded through OAA Focus on healthy living and disease prevention Evidence based – different levels Fit nicely with Medicare Preventive Services Promoting Medicare’s Preventive Services is the most common way MIPPA grantees work on focus area 3. Another way county programs work at preventing disease and promoting wellness is by offering “health promotion” workshops. These are partially funded through Title IIID of the Older Americans Act. All people age 60+ are to be served with these funds. Some programs/workshops are “evidence based” which means they are proven to be effective. Only high level EB programs can be funded with OAA money. Other classes/programs are also available but these are covered with other funding sources. Partnering with HP programs is a perfect fit. People who sign up for classes are likely to also be interested in making the most of their preventive health benefits.

Four Common HP Workshops The Wisconsin Institute for Healthy Aging coordinates these high level, evidence based programs: Living Well with Chronic Conditions Healthy Living with Diabetes Stepping On Powerful Tools for Caregivers List of upcoming classes is on their website: https://wihealthyaging.org/workshops You have probably heard of all these classes and hopefully they are being offered in your area. These are all highest level evidence based. You can search your county for a particular class, or search for a particular class in the state.

Living Well with Chronic Conditions Often just called Living Well 6 week class Targets people with arthritis, diabetes, heart disease, etc. Also for caregivers of people with chronic conditions Taught ways to live with their life-long conditions.

Healthy Living with Diabetes Responding to large number of people with diabetes and pre-diabetes 475,000 people in WI living with diabetes 6 week class

Stepping On Falls prevention workshop 6 week workshop Exercises – balance, strength, flexibility Vision and falls Medication side effects Safety at home and in community 6 week workshop

Powerful Tools for Caregivers For people caring for loved ones who are frail or have dementia Spouses, adult children, friends/neighbors 6 week class Qualification is different for Title IIIE – caregiver funds – does not have to be 60+ May not be on Medicare but likely are caring for someone who is

Other classes Strong Bones Tai Chi Arthritis Foundation Exercise Classes Lighten Up Med Wise Chair Yoga or other Yoga Beneficial Bites There are other health promotion classes that are commonly offered that are not high level evidence based, but are still beneficial and attract people interested in staying healthy.

Partnering with HP Coordinator Share target audience and message Physical Health + Financial Health = Overall Health Meet your county coordinator Updated list is on WIHA website https://wihealthyaging.org/workshops See MIPPA Partnership with Health Promotion for tips Educate each other about your programs Use Brief Program Descriptions Partner with your HP program coordinator to extend your reach and theirs! Target audience and message are same or similar – physical health plus financial health means better overall health. Make an effort to meet the coordinator and have a conversation about how you can work together on your outreach

Best ways to collaborate Communicate frequently about upcoming events Give Preventive Services information for class participants Possibly do a presentation before or after class (but not during) Give flyers for upcoming HP workshops to your clients Work together on outreach events and materials Share advertising ideas and costs Again, people taking HP classes will likely be interested in their Preventive Services benefits. There are several ways the HP coordinator and you can collaborate to share outreach materials/activities. You can also give other MIPPA information to share. Phys health + financial health = overall health. Leaders have to follow scripts for high level EB classes so won’t be able to talk about MIPPA info during class, but could have materials available before or after class. Be sure to share info about upcoming HP workshops when you are doing your MIPPA outreach, too. Take each others’ materials to any vendor events Consider sharing media advertising costs

There are a number of promotional materials on the website that can be used.
