System Overview «DICOM Network» Informational System was planed and developed for Medical and Diagnostically Institutions for collections, processing and visualization of medical images. This system covers all the workflow for processing and documentation of medical investigations from collecting investigations from medical equipment to archiving investigation in the patient medical record. «DICOM Network» offers necessary functionality for quality medical management and secure investigation access in ONLINE mode. This helps doctors, specialists and penitents to access, save and document Medical Imaging Investigations and help institutions to reduce investigation costs, enchasing quality of service.

Services & Features “DICOM Networks” informational system provides full set of functionality for data collection and processing. Data Collect – import DICOM investigations data from equipment Data Archive – full archive of investigations for several years Data Access – role based access to the investigation online with reporting and filters features. Visualization – Visualization of investigations in DICOM Viewer Data Processing – data preprocess and classification.

Numbers 1’st put into operation information system working in the RM, which offers distributed data collection, processing, access and archiving of medical images 3 DICOM Portals were configured in DICOM Network. 4 DICOM Servers are colleting DICOM data in 24/7 mode. 200 Investigations average per day are collected by the system now. 400 Investigations per day are expected by the end of the year when all equipment will be connected to the system. 496 Doctors have access to their patients investigations from their working place 500 GB of date is collected every month. 2 193 Times investigations were downloaded by doctors and specialists last 2 months. 1 278 960 MDL 62 098 euro were saved on the print consumables for investigations printing in 2016 thanking to DICOM Network.

DICOM Portal Front End of the Informational System Web portal provides end-user access to the system through an Web interface. Portal offers work space for following user roles: Patients who have access only to their investigations based on Patient ID. Doctors who have access only to their patients investigations. Doctors radiology specialists who are working in 24/7 regime. Users with limited time access, to get temporary access to specific research. This is necessary to ensure access to foreign experts, the legacy organizations, etc. Students & Researches have access to the anonymized datasets. System Administrator that performs monitoring and configuration of the system and «DICOM Server» and regulates the level of access for other users on the system. For administrators there are developed management console also available ONLINE.

DICOM Server Equipment integration and data storage This module provides a service of collecting data from the equipment, saving the archive of images and transmission of information on the server in the «DICOM Portal». This service is designed for various interfaces import / export of data, depending on the beneficiary's equipment. «DICOM Server» can also be used as a PACS and provides all the necessary commands for the protocol. Thus it can be used including adherence to the side of specialized Viewer system. This component is also responsible for the transfer of research to «DICOM Viewer» on the basis of appropriate access levels. This component can be scalability, and be installed in different locations and organizations, allowing you to store image archive distributed without impacting the concrete infrastructure of certain organizations.

DICOM Monitoring & Audit Equipment and services monitoring Performance monitoring system-configured «DICOM Server» s. This system allows you to determine the failure or unavailability of equipment connection, which in turn allows us to solve problem early on and increases the resiliency of the entire system. Personal data security and Investigations access. Each Investigation is linked to an patient with its personal data. For Each investigation there is possible to set individual access. Each request to data access is logged and investigation workflow is tracked.

DICOM Network as NREN service Why “DICOM Network” customers are interested in NREN services? Hospitals does not have enough resources to host the system and keep archive. Hospitals need qualified staff to maintain the system International connections Why NREN is interested in offering “DICOM Network” services? This offers new services that NREN can deliver to research and educational community. Large datasets that could be shared in international projects Application could be used for student education. Research activity Collected datasets could be used for research and scientific activities.

Integration into VISEEM Platform “DICOM Network” was selected as pilot application for integration into distributed regional VI-SEEM platform What “DICOM Network” could contribute to the research community of VI-SEEM Project? Archive of anonymized datasets were uploaded to the VI-SEEM server http://viseem.dicom.md/ offered by Macedonian partners. Impersonated metadata from integrated HIS. Mechanism for sharing the patient personal data, but only on patient approval. How VI-SEEM could help “DICOM Network”? Resources: storage, processors, etc... Promotion on international levels. Connections with other researches. What problems were solved for VI-SEEM integration? Architecture of the system was changed for fitting into VI-SEEM platform. Data anonymization mechanism have to be improved to be configurable on the DICOM Portal and institution level.

What's Next? Collecting large datasets provides possibility for data processing and classification based using “deep learning” and other algorithms. But first should be solved problem of data archiving The problem of data optimization for DICOM Network can be divided into 3 stages. First, when you import and write the source files on storage, you need to archive the data to minimize the volume, saving the quality of the images. Secondly, when accessing data, it is necessary to transfer the data to user as quickly as possible while optimizing the format to reduce the amount of transmitted images. Thirdly, the data should be optimized to speed up loading and processing them on the local processor. The bottleneck for future development is insufficient Storage!

What's Next? Future research, activities and innovations Mobile applications - a set of components and mobile applications that will allow the system to be used outside the workplace. These components will be useful in urgent medicine or in providing medical care outside of medical institutions. Optimization of data archiving - will significantly reduce the size of stored and transmitted data. Deep Learning for classification and automatic data processing. Automatic detection of pathologies based on previously processed datasets .
