Are the “Consignation électrique” practices homogenous between the machines? Are the Roles and responsibilities well established and understood for the process of “Consignation électrique” in injectors? Survey on "Chargés de Consignation" Homogenous practices? Are the “Consignation électrique” practices homogenous between the machines? Are the “Consignation électrique” practices homogenous in the PSB No, the “consignation électrique” practices are not homogenous in the PSB. It depends on the groups and the type of interventions If groups master their equipment and lock-out procedures, they have no knowledge of the electrical status of the other equipment  No overview of the electrical status of the PS Booster 24/06/2016 Survey on "Chargés de Consignation"

Roles and Responsibilities? Are the Roles and responsibilities well established and understood for the process of “Consignation électrique” in injectors? The “Roles and responsibilities” are PARTIALLY established and understood for the process of “Consignation électrique” in PS Booster. If Roles and responsibilities are not clear then the traceability of the Lock-out process is lost and people intervene without “Autorisation de travail” in a electrical machine.  CERN organisational structure is not equivalent to the roles and responsibilities defined by the standard NF C 18-510 which makes the implementation process difficult. 24/06/2016 Survey on "Chargés de Consignation"

Lock-out launch for the equipment owner (inspection, maintenance, reparations etc.) – Specific location Very usual  Well mastered, near what is requested by NF C 18-510 standard Lock-out launch for an other group intervention and not for the equipment owner intervention (Work distributed in different locations, external contractors etc.) Not so usual  More electrical roles involved. Roles and Responsabilities? “Chargé d’exploitation électrique” for injectors is not identified CdC “1ère étape” (First line and inside groups)  Well established CdC “2ème étape”  To be extended “Autorisation de travail” to be accompanied on field CERN should clarify roles and responsibilities in order to comply with the NF C 18-510. If roles are missing a solution adapted to CERN organisation should be identified and validated No overview of the electrical status of the PS Booster generates difficulties to manage the electrical co-activities on field. In addition, the traceability of the lock-out process is lost. 24/06/2016 Survey on "Chargés de Consignation"

Roles and Responsibilities? Could we define a reference person for the PSB and/or injectors who will do the follow-up and collect the different lock-out actions from the different equipment owners? (Overview) This person should have the knowledge and authority to sign the “autorisation de travail” (Traceability) Responsibility remains within each group, it should not be a substitute to ”Chargé de consignation” and “Chargé de travaux” present in each group 24/06/2016 Survey on "Chargés de Consignation"

Safety electrical Training CdC inside the groups : Around 50% are BR and/or HR (“consignation” for himself) and 50% BC and/or HC (“Consignation” for another group)  100% trained 50% are signed by the supervisor EDH “electrical habilitation” signature project in progress thanks to HSE and to DSOs Should be signed by the supervisor!
