WHY? 80% of post acute organizations are on Electronic Medical Records (EMR/EHR)-We already have data at our fingertips! Health Dept and Social Services already require tracking and trending (i.e. incidents, medication errors, discharges, wounds, fractures, anti-psychotics) We already know how through training and guidance! CMS requires MDS data and Quality Measures already in long term care/skilled nursing communities-it is already being used by our oversight agencies! -Zeigler study published on CAST website, survey of CFOS Well… Why not?!

Times are changing…where will you be? Affordable Care Act Higher Acuity Residents ACOs, MCOs, Performance Based Payments

Predictive Analytics-Data How is it important to providers? Reduce hospitalizations-proactively handles chronic disease management prior to crisis Increase quality-in care and customer service Cut costs-crisis always costs more, more labor, more expertise, more medical supplies Retain staff- assists staff with planning of resources, time and work schedules Position you for better marketing- better reputation=more business Decrease trauma in elderly transitions- reducing emotions of resident and family, displacement of confused residents when in hospital and scariness of being ill -costs-high hospitalization rate can be a financial burden to organization with new financial models of bundled payments

What can it affect? Powers to change? -it is a tool for us to utilize as providers to manage chronic disease, to improve the way we provide care, how aging seniors can feel safe and trust providers to take care of them, coordinate care and act on predictions to help prevent

If you choose NOT to utilize your data, what are the consequences? You will be stuck in the past while others move forward… Your competitors will utilize the new Technology… Your customers will demand it as a minimum service.. Your partners will expect it as a routine service.. Your payers will require it! Stuck in the past, others will start to use it, competitors will market it,

Predictive analytics could be the next new king of health care tools for providers… Outcomes, Quality, Cost Control Which can be intimidating, but that is why we are here today to help educate on what it is and how it can help you….the next step…
