Student Services Important Dates November 6 , 2017 Phone lines open to create an appointment. Please call 650-558-2251, email enrollment@smuhsd.org, online booking: https://priorityenrollment2018.youcanbook.me or come into the Office of Student Services to setup an appointment. December 5 Priority 1 Enrollment begins. All appointments will be taken first. Walk-ins are welcome but will only be served after appointments. February 16 , 2018 Last day for Priority 1 Enrollment & last day to apply for Intra-District Transfer or Sibling Transfer *If applicable March , 2018 Date to be announced Lottery drawing for all Intra-District Transfer requests. July , 2018 Summer Census (all Students are required to update emergency contacts, health, and mandated notifications.) Student Services Enrollment Information Where will the enrollment be held? Priority Enrollment will be held at the Office of Student Services, located at: 839 Hinckley Rd Burlingame, CA 94010 Do I need an appointment for Priority 1 Enrollment? Appointments will help reduce foot traffic as well as parking restrictions in our new location. Those who come without an appointment will need to wait. Those with appointments will be helped before walk-ins. Student Services REQUIRED DOCUMENTS Student’s Information: Birth Certificate and/or Passport Immunization Records showing TDAP Vaccination (If coming from Foreign, must be translated) Transcripts if enrolling from Private School or Outside of the District Custody Documents, as applicable Group 1 – Parent/Guardian Identification Valid Driver License with Vehicle Registration showing Current Address, Current CA ID, Current Military ID*, Current Passport,* or Consular Picture ID *Military ID or Passport may be used but must show additional ID with current address Group 2 – Residency Validation (one of the following) Current Rental/Lease Agreement or Renter’s Insurance Policy Current Property Tax or Homeowner’s Insurance Policy Shared Residency Affidavit** (Property Owner MUST also provide documents from all three groups) Declaration of Residency Form executed by Student’s Parent/Guardian Group 3 – Supplemental Residency Validation (one of the following) State or Federal Tax Returns with W-2 attached containing address within the district Payroll/Check Stub with name & address within the district Other form of identification or recent correspondence from a government agency to address within the district (e.g. Employment Development Department, Department of Social Services, etc.) Recent Voter Registration Card containing address within the district *The District reserves the right to request additional documents to determine residency or custodial issues.
