Multicultural "majority-minority" community with long-standing community health coalition committed to addressing the root causes of poor health seeks companion community. Must be willing to organize social justice marches, and testify at public meetings. Must like long walks, public transportation, and children's health. Looking for a foodie who loves farmers markets on Sundays, and is not afraid to get their hands dirty in the soil; must appreciate linguistic diversity, know how to involve community members and how to get politicians to pay attention. Areas of Focus for our SCALE Work Healthy Here: Communities Leading Healthy Change (CDC-funded REACH grant) Increase access to healthy foods Increase access to healthy, safe environments for physical activity Increase community-clinical linkages for chronic disease self-management Opioid Accountability Initiative Reduce opioid overdose deaths (heroin & Rx) Improve equity by focusing on Hispanic/Latino & Native American populations in the International District & South Valley
We are most proud of our work to… Align contributions in health equity-driven collective impact initiatives: Focusing on early childhood development through Pregnancy to 3 Task Force that promoted Nurse Family Partnership home visiting, resulting in federally-funded program in the South Valley, as well as contributing to the founding of the Early Childhood Accountability Partnership (ECAP). Working to reduce opioid overdose deaths by convening multiple sectors in long-term Opioid Overdose Accountability Initiative. Providing support over the years to healthcare access coalitions, as well as promoting development of ongoing programs to increase access (e.g., Pathways navigators).
