Bernalillo County, New Mexico . Multicultural "majority-minority" community with long-standing community health coalition committed to addressing the root causes of poor health seeks companion community. Must be willing to organize social justice marches, and testify at public meetings. Must like long walks, public transportation, and children's health. Looking for a foodie who loves farmers markets on Sundays, and is not afraid to get their hands dirty in the soil; must appreciate linguistic diversity, know how to involve community members and how to get politicians to pay attention. Our statistics are at the bottom, but our heart is at the top.

Top Three Sources of Pride in our Community Our people – pride in survival and self-determination of multicultural, long-standing communities co-existing with newcomers (for centuries) Our culture – arts, crafts, music, food, celebrations, fiestas, language Our land, water & sky – Sandia Mountains, Rio Grande, valleys, mesas, acequias, farms “This is ‘burque” https://youtu.be/IuKHGWvAxIg

Areas of Focus for our SCALE Work Healthy Here: Communities Leading Healthy Change (CDC-funded REACH grant) Increase access to healthy foods Increase access to healthy, safe environments for physical activity Increase community-clinical linkages for chronic disease self-management Opioid Accountability Initiative Reduce opioid overdose deaths (heroin & Rx) Improve equity by focusing on Hispanic/Latino & Native American populations in the International District & South Valley

We are most proud of our work to… Align contributions in health equity-driven collective impact initiatives: Focusing on early childhood development through Pregnancy to 3 Task Force that promoted Nurse Family Partnership home visiting, resulting in federally-funded program in the South Valley, as well as contributing to the founding of the Early Childhood Accountability Partnership (ECAP). Working to reduce opioid overdose deaths by convening multiple sectors in long-term Opioid Overdose Accountability Initiative. Providing support over the years to healthcare access coalitions, as well as promoting development of ongoing programs to increase access (e.g., Pathways navigators).

Our Community Theme Song….   If we choose to treat the illness But we just ignore the cause Thinking it will change disparity Others will continue dying Poverty will take its toll That is just not fair - inequity Inequity, don’t let it be, let it be equity We can make a difference – can’t you see   Let it be equity, let it be equity We can make it happen – equity  * Lyrics by Marsha McMurray-Avila, with help from John Lennon Let It Be – Equity* (to the tune of “Let It Be”) When some are healthier than others Graphs and charts are what we see Numbers on the page – disparity   But when we notice it’s not right They’re dying young ‘cause they’re not white Then it’s just not fair – inequity Disparity, inequity, inequity, disparity Yes, there is a difference – can’t you see

Celebrity Spokesperson Who Best Represents Us DOLORES HUERTA! A strong woman, born in New Mexico, who said in 2012 upon receiving the Presidential Medal of Freedom: “The freedom of association means that people can come together in organization to fight for solutions to the problems they confront in their communities. The great social justice changes in our country have happened when people came together, organized, and took direct action.” ¡Si se puede!
