Medicare is a complex federal health insurance that covers more than 58 million Americans who qualify by: Being 65 years of age or older Permanently Disability Having End-Stage Renal Disease (ERSD) Amyotrophic Lateral Sclerosis (aka Lou Gehrig’s Disease) Exposure to Environmental Health Hazards

What do I get if I have Medicare? Medicare has four different parts: Part A = Hospital Part B = Medical Part C = Part A + Part B +/- Part D Part D = Outpatient Prescription Drugs

Medicare Part D This is where we come to play ↓

How do we help? Medicare Part D Plan Optimization ~80% can save money through our service average savings $1,030/patient Medication Therapy Management (MTM) Evaluates medication for safety and effectiveness Increasing patient understanding and appropriateness of therapy Over a dozen health screenings and services

Medication Therapy Management (MTM) Services Comprehensive Medication Reviews Drug-Drug Interactions Therapeutic Duplications Side Effects Medication Adherence And so much more…..

And what do you do if you find a severe medication-related issue We communicate with the patients prescriber on behalf of the patient to communicate any issues we see and make recommendations as health care professionals

So once we address this issues how do we help patient compliance & understanding?
