Precautions Process Overview Process Overview (worktable manufacturing for bridge construction ) Material Receipt Cutting Welding Shot Printing Shipping Fatality Process Working processes Cutting: cut materials such as H beam at a regular standard using oxygen cutter Assembling: structural members were temporary assembled using a using ceil crane and then main welding was conducted. Shot: remove foreign material such as rust on the products with a shot blast machine Painting: paint the products with a spray gun ※ The bridge worktable was assembled but not painted for shipment. ※ Bridge worktable: bridge worktable is for works on the water surface while they inserting stake foundation on the ground for a bridge construction in the water. Fatality Object Fatality Object Fatality Object: bridge worktable Dimensions: 2,500㎜ × 9,800㎜ × 3,000㎜ They connect three bridge worktables manufactured in a factory into one set in bridge building sites. I. Fatality Overview II. Process Overview III. Fatality Object IV. Fatality Process V. Fatality Cause VI. Precautions IV. Fatality Process March O, OOOO, the victim went to the site at 08:00. 1 team of 4 workers assembled bridge worktable from 08:00 to 12:00. Morning work was mainly welding on the workplace floor and co-workers conducted welding on the upper part. They had lunch and took rest between 12:00~13:00, and started afternoon work from 13:00 and took rest again for about 10 ~ 20 minutes at about 15:00. At about 16:30, the victim, alone, was working on the upper part of the bridge worktable. He performed welding on the beam connections in the middle of upper part of the bridge worktable and took his face shield and welding gloves off. He was walking along the beam and missed his feet to fell down to the ground to death. Power Point Training Material on Serious Accidents in Production 7

V. Fatality Cause No protections against fall No safety equipment I. Fatality Overview II. Process Overview III. Fatality Object IV. Fatality Process V. Fatality Cause VI. Precautions V. Fatality Cause No protections against fall Even though the place to manufacture bridge worktable that is used for bridge building at high place above 3m with fall hazard, there were protections against fall such as scaffold, safety harness. No safety equipment - Even when he was working on the upper part of a bridge worktable that was a high ground working place above 3m with a fall hazard, he was not wearing a safety cap. VI. Precautions Thorough protections against fall I. Fatality Overview II. Process Overview III. Fatality Object IV. Fatality Process V. Fatality Cause VI. Precautions VI. Precautions Thorough protections against fall To work on the upper part of a bridge worktable that was a high ground working place above 3m with a fall hazard, you must install footing such as scaffold around the bridge worktable to secure workers’ safe path. In addition, you must install a safety net under opening between upper beams of bridge worktable. When a scaffold and safety net cannot be installed, you must install protections against fall such as safety harness attachment on the outer upper part of the bridge worktable. Wearing safety equipment Since there is a fall hazard and head injury while working on the upper part of a bridge worktable, you must wear safety equipment such as safety cap and conduct thorough management and supervision.
