HOUSING STATUS REPORTS

Disband Coordination Committee Present new bylaws and policies and procedures to Policy Council Craft new Mission Statement Sign 2015-2016 MOU with United Way and funders C3H will clarify its structure and have revised bylaws by February, 2016.

Approach Lompoc Valley Medical Center with proposal to support C3H by September 30, 2015. Reconsideration in Santa Maria Receive Funding from Marian Hospital Quarterly reports to funders Approach outside foundation with clear proposal. C3H will receive funding from all cities, three hospital systems, and at least one outside foundation by June 30, 2016.

Take over the management of and responsibility for the VISPDAT tool Coordinate all data requests, manage data updates, and develop reports and reporting formats by September 30, 2015. Link the VISPDAT tool to share and/or integrate the County's Homelessness Management Information System (HMIS) by March 31, 2016. Coordinate with hospitals on data sharing and outreach efforts by March 31, 2016. Spearhead a data sharing and consolidation initiative that will integrate data on people who are homeless and become a robust data provider and warehouse by March 31, 2016.

By March 31, 2016, C3H will coordinate the development of a cost analysis By June 30, 2016, project the net cost savings to the County if currently homeless people were housed. C3H will determine the actual cost of homelessness and the benefits of housing people who are homeless by June 30, 2016.

Ensure HEARTH Act implementation CoC becomes primary body for regional coordination of activities Implement coordinated entry system Conduct stakeholder survey on needs of providers Asset/Gap analysis and interactive mapping Veteran homeless targets reached C3H and Continuum of Care will become hub of regional activity, collaboration and coordinated data and assessment.

Clarify, dispel myths, and promote housing first strategies for chronically homeless and veterans. Clear, measurable outcomes in two regions by June 30, 2016. Foster solutions such as landlord-liaison and placing housing ready families with measurable results countywide. Implement data-driven, evidence-based solutions to homelessness in partnership with service providers, businesses and community leaders in a minimum of two regions by June 30, 2016.

➢ C3H will create a bi-weekly newsletter (or social media blast) for email distribution and website posting by September 30, 2015. ➢ C3H will retool and upgrade the look and feel of its website by January 31, 2015. ➢ C3H will realize a 25% monthly increase in its web page hits by March 31, 2016. Using a combination of social media, the C3H website, and email, C3H will become the voice of programs, services, events, resources and news about solutions to homelessness by March 31, 2016
