Who? I’m OK, You’re OK? Over 18? Every time you go to the hospital you will be asked for your living will. Everyone is asked; few answer correctly! Why? 65% of Americans do not know what it is or the consequences of not having one!

If I Am Living, Why Do I Need a Will? There is a half-life that can occur in a hospital setting--a coma, temporary or prolonged unconscious state. The Durable Power of Attorney for Healthcare says who can speak for you when you cannot. Your Living Will tells them what to say.

What is a living will? A document that states your specific wishes when you are hospitalized. It may be called an Advanced Care Directive. -Everyone ought to have one -It is effective immediately and until changed -It may contain a DNR (Do Not Resuscitate)

Your clients do not know the need unless… They have… Experienced it personally Read an article Watched a television show Heard from a friend Talked with a Dr. or Attorney

WHY? You want… -to be in charge of your treatment -your family to know your wishes -physicians to follow your wishes -to limit treatments that are unecessary -to end procedures that may cause harm -to get off the conveyor belt of care …and so do your clients!

Two New Documents MOST (Medical Order for Scope of Treatment) POLST (Physicians Order for Life Sustaining Treatment) DNR and AND conversations. These are designed to supplement an Advanced Care Directive. They are legislated forms to force ACP but not widely known or used.

What’s the Problem? MOST (Medical Order for Scope of Treatment) POLST (Physicians Order for Life Sustaining Treatment) are both limited in scope. Only for those with chronic, life threatening illness, advanced, critical or terminal illness. It must be reviewed every year and signed by Dr, and patient or agent. It is portable and must be placed in every patient’s file. Any blanks are assumed to be completed and accepted. It includes 12 statements. It must be printed on PINK paper. It allows selection of full treatment, limited or comfort care.

Complicated??? No, not when you ask the right way… “Who would you like to speak for you if you could not speak for yourself?” “What would you like them to say?”
