Background Studies have documented high burden of HIV among MSM in low- and middle-income countries In Nigeria, an increasing HIV prevalence among MSM has been observed in the last two rounds of Integrated Biological & Behavioral Surveillance Survey (IBBSS) (13.5% in 2007 & 17.2% in 2010) Behavioral roles among MSM are important factors in HIV spread Little is known about HIV infection attributable to sexual behavioral roles among MSM in Nigeria This study examined correlates of HIV infection among MSM in six states in Nigeria

Methods We analyzed data from the second round of IBBSS Recruitment network drawing We analyzed data from the second round of IBBSS Between April – May, 2010, 1, 545 MSM aged 15-49 years were recruited through the Respondent Driven Sampling (RDS) technique in six Nigerian states A structured questionnaire was administered through face-to face interviews Blood specimen collected and tested for HIV Multivariable logistic regression was implemented in STATA to determine correlates of HIV infection

Methods SOCIO-DEMOGRAPHIC FACTORS Age, educational status, marital status HIV-RELATED KNOWLEDGE HIV awareness, knowledge of HIV prevention methods, risk perception of HIV SEXUAL BEHAVIOUR Sexual positioning, condom use, partner types EXPOSURE TO HIV PREVENTION INTERVENTIONS HIV testing, condom distribution, HIV education OUTCOME HIV infection

Results 1,545 MSM Median age=25 years 51% bisexual Commercial partners (bought sex) 344 (22%) Consistent condom use 96 (28%) Commercial partners (sold sex) 555 (36%) 176 (32%) Non-commercial partners (non-paying) 1,001 (65%) 314 (31%)

Results HIV TESTING AMONG MSM 100% 83% 17% MSM sampled (N=1,545) Tested for HIV (n=1,288) 17% HIV positive (n=222)

Results Factors associated with HIV infection were: Older age [25-35 years: (AOR=1.56, 95% C.I (1.07 -2.28)], Reporting receptive anal sex [AOR=2.15, 95% C.I (1.39-3.32)], Having multiple male partners [AOR=1.78, 95% C.I (1.03-3.06)], Knowing an HIV infected person [AOR=1.56, 95% C.I (1.06-2.29)]

Conclusion This study showed a relatively high prevalence of HIV and sexual risk behavior among sampled MSM Inconsistent condom use indicates that HIV transmission among this group is likely to continue unabated and thus must be addressed Differences in HIV prevalence amongst MSM engaging in receptive and penetrative anal sex suggests targeted behavioral and biomedical interventions tailored to the specific sexual roles are urgently needed
