Athletic Packet 101 Athletic packets are required to maintain eligibility for athletes. Athletic packets should remain stapled in page number order. No athlete can be eligible for play without this packet! Please review this presentation to ensure that your packet is completed correctly.

Page 6 – Physical (3 of 3) This page is only completed if the primary physician does not clear the athlete. Athletes should be referred to a specialist for re-evaluation and clearance.
