Why is PGx Testing Important Why is PGx Testing Important? Drug Related Adverse Events Impact Patient Care and the Cost of Healthcare Rx related AEs cost the US healthcare system ~$136B/YR 6-7% of hospitalizations due to Rx related AEs. ADRs cause 1 out of 5 injuries or deaths per year to hospitalized patients. AEs cause ~100,000 deaths/year (4th leading cause of death). >2.2 million serious adverse reactions/year. Mean length of stay, cost and mortality for ADR patients are DOUBLE that for control patients.

INGENIOUS (INdiana GENomics Implementation: an Opportunity for the UnderServed) PI’s: Paul Dexter, MD Todd Skaar, PhD The INGENIOUS trial (NCT02297126) is sponsored by an NIH/NHGRI U01-grant (HG007762) As you know, the initial grand was submitted to the NHGRI with Dave Flockhart and Paul Dexter as the co-PI for the study. With Dave’s passing, the NHGRI accepted our recommendation to add Todd Skaar as the second PI to the study.

INGenious Overview Study Aims: Collaboration: Indiana University School of Medicine Eskenazi Health System Indiana University Institute for Personalized Medicine Regenstrief Institute Study Scope: 2,000 patients in study arm 4,000 patients in control arm Study Aims: Aim 1: To test the hypothesis that a CLIA certified genotyping targeted at 28 widely used drugs is associated with significant reductions in hospital and outpatient costs incurred over a one year period Aim 2: To test whether pharmacogenetic testing is associated with significant improvements in clinical outcomes over a one year period IU’s U01 INGenious study is a collaboration between the IU SOM, Eskenazi Health, IIPM and the Regenstrief Institute We have 2 study aims

INGENIOUS PGx Study 16 genes and 51variants validated on custom open array in IU’s CLIA certified pharmacogenetics laboratory The INGenious study design includes 15 genes and 51 variants on an custom open array performed in Vicky Pratt’s CLIA certified PGx lab

PGx EMR Alerts Created by Clinical Decision Support Rules Pharma Pharmacogenomic Alert! This patient has pharmacogenomic information that may impact this prescription Medication: Clopidogrel Gene(s) involved: CYP2C19 Phenotype: Poor Metabolizer Recommendation: Consider an alternative antiplatelet therapy, e.g. prasugrel, or ticagrelor. This patient’s poor metabolizer status predicts poor clopidogrel efficacy. The Drug-Gene Pair committee that meets on Tuesday mornings has evaluated all of our target medications, potentially actionable results based on the patient’s PGx report and has developed alert “pop-up” that will occur after a subject is enrolled, genotyped and a provider writes a new script for one of our targeted medications. This will provide the prescriber instant notification that there is a clinically relevant drug-gene concern and provide alternate therapy recommendations. INGENIOUS PGx Report Page 1 When a subject is randomized into the study arm and they are consented and enrolled, their PGx report is entered into the EMR. The first page provides the clinician with the specific medication that created the alert and provides the PGx result and the interpretation.

INGENIOUS PGx Report Page 2 Genotype/Phenotype Results Page 2 of the report has the full panel results. We are in the process of modifying how this page in displayed so that we highlight the significant genotype/phenotypes but not overload the clinicians with data that they may not want to see. It is a concern that data overload may be a negative and hinder PGx adoption or acceptance for PGx panels.

This process is intended to review the patients genotyping results, scan their active medication record, add relevant PGx finding to the Problems list in the EHR and alert the clinical pharmacologists on-service if it is determined that a consult with the prescribing or attending physician may be needed. Are We There Yet? So, a question I am asked quite often. Are we there yet or are we recruiting at a rate that we allow us to reach our target goal.... Not quite, but we continue to make alterations to the protocol to improve recruitment and drive towards our goal. Not Quite, we must continue to align academic research and the IVD industry to expedite adoption of new technology
