SAFE WORK INSTRUCTIONS Title of Subject: Hand Drills (Cobra, Hilti, Gopher, etc.…) Last Reviewed Date: November 2015 SAFE WORK INSTRUCTIONS Steps for Initial SWI Training: Prepare – Review the SWI. Present – Quick Card at the machine with experienced operator. Tryout – Person runs the machine with supervision. Follow up – Competency points are verified by supervisor and 5000-23 is signed by the trainee to complete the training. Steps for Retraining: Review SWI or Quick Card Verify Competency Points Complete Annual Training Requalification Form Competency Points: Pre-op © Drilling horizontal and / or vertical holes© Installing bolts into roof and ribs©   The following safe work instructions is NOT INTENDED to replace hands on training requirements of the code of federal regulations. The intent is to provide an overview of known potential safety hazards associated with equipment operation. Hands on training with an experienced operator on each specific model must be conducted. The trainee can not proceed onto production until they are determined ready to proceed by the supervisor of the training. Before any operation or maintenance procedure is performed with or on this vehicle, it is important that personnel have a complete understanding of the location and functions of all controls and indicators, as well as, the location and use of all emergency shutdown controls and procedures. SAFE WORK INSTRUCTIONS Title: Hand Drills (Cobra, Hilti, Gopher) KEY POINTS SAFETY PRECAUTIONS Work Area Examination: Examine the roof and ribs for loose materials and the walkways for potential slipping/tripping hazards. Scale loose roof and ribs as necessary. Remove potential slipping/tripping hazards. Pre-ops: Do not allow walkways to become obstructed from the work activity Test all levers to ensure proper operation. Examine hoses for leaks, fraying, or damage and hose connections and joints for leaks or damage. Maintain bolter free from accumulations of combustible materials.(Coal, dust, paper, grease and oil). Scale loose roof and ribs as necessary. Remove potential slipping/tripping hazards prior to beginning task. Levers must not stick Remove any debris or accumulations detected at the bolter. Hearing protection is required to be worn when working inby the fan location or last open crosscut. Ensure that all personnel are in the clear prior to starting the machine for any of the checks. Frayed or damaged hoses must be repaired or replaced prior to operation. Ensure that there is a slate bar or other suitable device for scaling roof and ribs in the area. Always communicate your intensions with all others in the affected area. Do not place hands on top of the chuck.

SAFE WORK INSTRUCTIONS Title: Hand Drills (Cobra, Hilti, Gopher) KEY POINTS SAFETY PRECAUTIONS Drilling: Monitor the workplace for changes during the work activity. Examine drill steels for burrs and straightness. Measure drill steels to ensure proper length and properly marked. Utilize proper body positioning and hands off drilling. Ensure that hole is drilled to the proper depth. Utilize a safe speed and feed for optimal control. Be alert for drill binding and or twisting. Utilize radios and cap light signals for effective communications between co-workers. Follow the site specific approved roof control plan for bolt spacing. Utilize proper tool for scaling roof/ribs. Remove potential slipping/tripping hazards prior to beginning task. Drill steels must be straight and free from damage or defects. Maintain sharp drill bits. Never mark steels by scoring. Never place hands on steels that are rotating or being raised. Do not leave drill steels unattended in the roof. Store drill steels in proper locations to prevent becoming projectiles. Ensure that all personnel in the area are aware of your intentions. Do not start rotation until bit is in contact with drilling surface Hearing protection is required during drilling. No loose clothing Maintain firm grip while rotating drill.

SAFE WORK INSTRUCTIONS Title: Hand Drills (Cobra, Hilti, Gopher) KEY POINTS SAFETY PRECAUTIONS Installing Bolts: Examine the bolts and couplers for damage or defects. Examine bolt wrenches for defects and compatibility with bolts. Utilize proper body positioning and hand placement. Utilize a safe speed and feed for optimal control. Check the torque of the bolt with a torque wrench to ensure that bolts are properly torqued within range of approved plan. Check resin for condition and expiration date. Utilize safe lifting techniques when handling materials. Do not use damaged or defective bolts/wrenches. Wrenches must be manufactured. (No homemade wrenches). Do not place hands on bolts that are being raised. Store bolts and bolt wrenches in proper locations. Do not use resin that is damaged or expired.
