Primary/ Secondary Care Joining up TECS and home care services can make a significant positive impact …. Home Care TECS Adaptations Primary/ Secondary Care …. but currently such ‘innovation’ is confined to small-scale pilots

Of course technology can help provide more efficient and effective home care services … Real Time Transform Proactive intervention Consistent scheduling Design home care services Understand immediate needs Engage family… in a way that works Personalised care planning Future service integration Safeguard against risks Population health management Mental health services … and there’s huge potential value within the data that is created and collected

The changing market also demands us to think and act differently …. Technology Enabled Care … Technology could be built into homes to ensure people don’t leave unaided – eg wearable tracking devices Patients to see the same GP via Skype Reduces hospital admissions Consistency of care workers Template schedules based on frequency Care Workers log visit information electronically to ensure the information is easily accessible to review Accessible via health professionals and family Care Workers could support individuals to do her grocery shopping online Ability to chat online to combat isolation and loneliness Family engagement PHBs and DPs Consumer market More demanding Integrated services … and small changes can lead to big outcomes
