Dementia in the acute setting The role of Dementia case manager

My role Support patients living with dementia whilst in hospital. Advise and support; carers, relatives and staff when needed. Provide training to all staff members. Provide specialised input when dementia is advancing and symptoms worsen. Implement new policy and procedure. Introduce campaigns, pathways and strategies. Improve links within the community (professionals, patients and carers).

My role cont…… Complete capacity assessments and assist with DoLS or refer for MHA Dementia screening as part of our KPI Attend safeguarding meetings

So far…… We have improved the environment on our care of the elderly ward Bought 4 large computers on wheels with DRTS and 2 small android tablets Trained dementia champions (6 month accredited course) Established Tier 1 and Tier 2 dementia training Consistently achieved 90% and above for dementia KPI Worked together with Hospedia to create bedside orientation screens Improved signage Set up a dementia steering group Have regular groups running e.g. Arts and crafts, music and gardening

What’s to come? We will be launching John’s campaign in the next month A new admission pathway for patients living with dementia will be initiated (identification scheme) Further champions to be trained As of July our hospital cinema will host the Monday afternoon matinee, this will show a mixture of new and old films Further work will be done with the environment Dementia carers group ? To be formed
