Improving Health Equity: Short- and Long-Term Solutions Panel Session – Friday November 3, 2017 Deborah Paone (Facilitator) cara james, amy helwig, ana fuentevilla Through our panel presentations and interactive discussion with all present, we will focus on key issues in improving health equity for vulnerable populations, including: SDOH data availability and completeness for identification, stratification, reporting Differences between urban and rural populations and areas – impacting strategies Working with community partners and other stakeholders Resource issues and other challenges and how to move ahead effectively

Key questions & facilitated discussion Metrics to Address Health Equity and Track Progress Are you finding that there are key success strategies or effective community collaborative approaches emerging—based on qualitative or quantitative evaluation of progress? What metrics and methods have you or your partner/collaborator organizations put forth to evaluate progress around these collaborative efforts in addressing health equity issues?

How would you advise organizations that want to move ahead with efforts to identify/ define vulnerable populations groups with high social determinant of health needs?
